Castville Solutions, on behalf of our reputable healthcare clients, is recruiting a Community Health Worker to promote health awareness, support community-based healthcare initiatives, and bridge the gap between healthcare providers and local communities. This role is vital in improving access to quality healthcare and fostering healthy living within communities.

Key Responsibilities Include:

• Conduct community outreach and health education on topics such as hygiene, nutrition, immunization, and disease prevention.
• Support individuals and families in accessing healthcare services and resources.
• Assist healthcare professionals in implementing community health programs and interventions.
• Identify community health needs and report findings to supervisors or healthcare coordinators.
• Collect and record data on community health indicators for reporting and evaluation purposes.
• Provide follow-up support to patients and ensure medication adherence.
• Encourage community participation in health campaigns and vaccination programs.
• Promote awareness of maternal and child health, family planning, and environmental hygiene.

Qualifications:

• Certificate, Diploma, or Degree in Community Health, Public Health, or a related field.
• Minimum of 1–3 years’ experience in community health outreach or a similar role.
• Strong communication and interpersonal skills.
• Passion for improving health outcomes and serving communities.
• Ability to work independently and as part of a team.
• Familiarity with local languages and cultures is an advantage.
